I frequently reverse Sword and Long Sword when I'm trying to use Dream Sword...needless to say it's not useful against the SP Navis. -_-;

In EXE 3 i was about to destroy Forte GS and i had all the chips for bodygaurd but i acciodently used the add button and lost the chips and then the next turn Forte killed me.

I was playing Soul Calibur 3, and was helping a friend against that annoying girl with the staff, when she gets behind me, and I pushed her off the ledge. Literally. XD
Another happened while I was playing Maverick Hunter X, Armored Armadillo's stage (all the bad stuff happens in there >.>), and I was at the very last part, and I jumped off the cart-thing and it hit me in the back, pushing me through the door so I was stuck at the pain sprite while going through. >.<

Wow, that was the ultimate comeback 
I think one of my worst mishaps was when I was playing Boktai 2, I was standing on a solar panel (the one that only appears with the right amount of sunlight) and a bunch of boks came near me. I was like Damn Damn! So then in my frustration I covered the solar sensor and we all fell down into the hole. I forgot to save since the past piledriver :<
