Almost all the characters from "Gundam Seed", actually.
Damn artist...
I was quite baffled by the relationship between Loran and Lora before I watched Turn-A Gundam, but it was clear that they are one in the same. Cross dressing characters... 
Oh yes, and there's Doris from Akazukin ChaCha, though the series made it clear that it's a guy. XD |

I know one. That one guy from Cowboy Bebop. It was the episode where Fei was talking to a guy in a shower. I know there was something amiss about that whole thing. (If someone knows what im talking about please say so. XD)

He was messed up by chemicals, during some kind of battle, when he and Spike knew and hung around.
The chemicals changed his body's gender

Some people thought that Chris from King of Fighters was a girl...
I'm gonna go with the majority and say Jakotsu, Kamatari, etc. But I'm also gonna throw in Flea(Chrono Trigger/Cross), Tsubasa Kurenai(Ranma 1/2), RhodoKnightmon(Digimon Frontier), and Ranmaru Mori(Samurai Warriors).
Personally, I pretty much could tell Narcy was a guy.
But what about girls who people think are guys at first?
The first time I ever saw Akane Tendo(Ranma 1/2) was in a magazine ad for the Hard Battle SNES game, and I thought she was a he.
Also Makoto from Street Fighter 3. I thought she was a guy until I actually saw her ingame sprite.
Oh, and Hilde Shibeker from Gundam Wing. I could have sworn up and down she was a guy.
Lastly, Kojiro Sanada from The Last Blade 2. But then again, she was TRYING to look like a guy.

I would have to say Alister(is that right?) from Yu-Gi-Oh. He almost looked like a girl, and I tought he was until i heard his voice.

Well, she really was attempting to pass as a guy in the first Ryuuko no Ken(Art of Fighting in the US) game. After that, she still dressed kinda masculine, but it was easy to tell that she was a girl.
